35 Job Description DEFINITION Under close supervision, incumbents will primarily perform wellness services within a multi-disciplinary treatment team setting.
Wellness Coaches work with consumers to determine their problem areas, identify obstacles, and work with the treatment team to create a plan to achieve their health and wellness goals.
Additionally, incumbents will teach clients about eating, healthy exercising properly and the eight domains of wellness.
The Wellness Coach will keep clients motivated and on-track to overcome challenges.
EXAMPLES OF WORK PERFORMED Utilize active listening and problem solving skills and/or Motivational Interviewing to assist consumers to identify barriers to recovery and to assist in the development of a plan to meet determined goals.
Provides culturally competent support and trauma-informed care in order to maintain a safe, respectful and therapeutic environment.
Collaborate with consumers and care teams to provide support and increase knowledge and understanding of health and emotional conditions.
Work with consumers to increase knowledge of health and emotional conditions.
Assists consumers in understanding goals related to team recommendations.
Supports focus on wellness promotion and provides education regarding preventative care measures in collaboration with the primary treatment team.
Provides screening support in order to provide necessary data to appropriate care team members for referral and treatment (examples: 5A’s, PHQ-9, etc.
).
Instills confidence and supports consumers to engage in self-management skills.
Assist with the development and management of the 8 domains of wellness.
Supports consumer in identifying self-management goals and developing customized plan with care team including goals and a plan of action.
Encourages consumer to participate in shared decision making with multi-disciplinary care team to enhance adherence to treatment plan and medications schedules.
Works with consumer and team to identify barriers to wellness.
Manages accountability and progress with consumer in follow up phone calls, and with individual or group coaching sessions.
Assists with consumer referrals to appropriate level of care for mental health and substance abuse treatment.
Participate in DMHSAS-sponsored Wellness Coach Training Program.
Performs other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES Knowledge of the English language in order to communicate effectively, both orally and in writing.
Ability is required to record observations and activities, to follow oral and written directions, and establish and maintain effective working relationships with others.
Basic computer skills are required.
Knowledge is required of trauma-informed, mental health and/or substance abuse prevention treatment and/or recovery programs or goals; ability to establish an effective, collaborative relationship with a consumer, work effectively with a treatment team and maintain healthy boundaries with consumers.
This position provides care to a vulnerable population which requires employees to adhere to all applicable laws, rules and regulations, including but not limited to consumer rights regulations, confidentiality laws, and accreditation requirements associated with this position and field.
As such, incumbents should be proactive, detail-oriented, and diligent with protecting consumer information, and possess the ability to understand all laws and regulations regarding consumer rights.
Incumbent must demonstrate honesty and integrity in their actions and communications regarding issues impacting the organization.
Incumbents must demonstrate a commitment to serving the citizens of Oklahoma, including, but not limited to demonstrating compassion in their care of ODMHSAS consumers, focus on achieving positive treatment outcomes, responsible fiscal management, and support for the ODMHSAS mission to serve individuals and families impacted by mental illness and addiction.

driver’s license at the time of appointment.
Willing to fulfill any job related travel normally associated with this position.
